**Medical History/Physical Exam Form**

**Certificate of Immunization/TB Questionnaire**
Students must also provide proof of up-to-date immunizations and complete a tuberculosis (TB) screening questionnaire. Commonly required vaccinations include MMR and COVID-19, as well as TB tests.
**Consent for Medical Treatment of a Minor**
If the student will arrive at Oberlin prior to their 18th birthday, a Consent for Medical Treatment of a Minor form is required. This form allows medical personnel to provide treatment to minors should they require it.
**Submission Procedure**
Once the forms are completed, students should log into the Student Health Portal using their college credentials to upload them. A copy of the insurance card is also required for submission. The submission of forms and insurance card should be done after the physical exam.
It's important to note that the Medical History form is required, and it consists of pages 1 & 2. The Certificate of Immunization form requires page 4, including the TB Questionnaire. The physician will complete the remaining parts of the forms.
The physical exam should be conducted within 12 months prior to the start of the academic year, and the completed forms, along with a copy of the insurance card, should be submitted via the Student Health Portal.
