In line with the Black-Red coalition's ambitions, health care for Germans will primarily lead to family doctors who will subsequently refer patients to specialists when necessary. However, the National Association of Statutory Health Insurance Physicians (KBV) advises the coalition against a blanket adoption of a primary care system for statutory health insurance patients.

Andreas Gassen, KBV chairman, urges the coalition to approach this system selectively, suggesting it is more suited for older patients. "Around 50, it makes sense. By then, many people are already in medical treatment for various health issues," Gassen underscores.

The prospective primary care system, as envisioned by the Black-Red coalition, dictates that family and pediatricians will manage patient referrals, except for eye doctors and gynecologists.

Gassen champions the notion that such a system can work for older multimorbid patients in need of an organizing hand to manage multiple diseases concurrently. He also advocates for more sustainable self-participation, such as encouraging patients to consult family doctors and avoid visiting specialists of their personal choice.

Dahmen: Encourage Support, Not Obligation

Green health expert Janosch Dahmen backs the steering concept, but also appeals for relief for family doctors. To avoid needless doctor visits, long waiting times, and disjointed processes, Dahmen recommends more time through advanced payment allowances, stronger support through non-medical practice staff, and a networked, digital appointment management system.

Dahmen cautions that the risk of real undersupply looms if mandatory primary care is introduced without ensuring sustainable solutions, particularly in rural areas where family doctors are scarce.

The Black-Red coalition receives support for its primary care physician model from the Association of General Practitioners. Nicola Buhlinger- Goepfarth, its chairwoman, asserts that implementing the system could mean an additional two to five patients per day for each general practice.
